PLAINFIELD, Ind. -- Amazon is facing a $28,000 fine after a worker at their Plainfield warehouse was killed by a forklift earlier this year.
A safety order issued by the Indiana Department of Labor says the September 24 accident was a result of inadequate training and failure to follow safety procedures by the Amazon warehouse.
Phillip Terry, 59, was crushed while working on a forklift at the distribution center at 800 Perry Road. The forklift weighed over 1,000 pounds.

The notification orders Amazon to correct their actions and pay a $28,000 fine.
