Amazon employee killed in forklift accident at Plainfield facility

PLAINFIELD, Ind. -- The Amazon employee who died in a forklift accident Sunday at the company's Plainfield facility suffered multiple crushing injuries before his death.

The accident happened around 2:30 p.m. at the facility at 800 Perry Road.

Plainfield Police Captain Jill Lees said officers were dispatched to the scene, where they found paramedics already working on 59-year-old Phillip Terry from Indianapolis.

Ultimately, Terry was pronounced dead at the scene. 

Lees said Terry had injuries to his head and torso, including multiple skull fractures and brain injuries.

Lees said the investigation was still very preliminary, but that police were treating the death as an accident.

Amazon released a statement on the incident:

Our thoughts are with our associate's family and loved ones during this very difficult time. Safety is our number one priority, and as we do with any incident like this, we will be reviewing our practices and protocols to ensure the well-being of our employees.
